How do I terminate?

In order to terminate, you must submit a request in writing including the following: date, name, social security number, effective date of termination (which must always be the last day of the month), and signature. Fax the letter to Fractured Atlas at 212-277-8025.

A request for termination must be received no later than the 1ST of the effective month of termination. For instance, a request for termination as of August 31st, must be received no later than August 1st.

Terminations cannot be made retroactively and we are unable to issue refunds for coverage.
