The Fine Print (in a reasonably large font)
These plans provide "individual" insurance, not "group" insurance. The legal reasons for this are complex, but for the most part it's because most state insurance laws recognize employer groups only.
The difference is that we're a non-profit organization whose mission is to support artists and creative professionals.
How much a plan costs, and whether your application is approved or declined, are based on individual factors. These include your age, zipcode, and medical history. Health insurance plan rates are set by law and are the same whether you obtain coverage directly, or anywhere else.
You'll pay the same amount for Aetna coverage through Fractured Atlas as you would if you applied directly or through a broker. The difference is that we're a non-profit organization whose mission is to support artists and creative professionals. You'll also have our support as an independent organization with one-on-one feedback and communication with knowledgeable staff. That's right: real, live, breathing people.
These plans also do NOT provide coverage for maternity or mental health care (except where it is required by law in the case of biologically based disorders). If you absolutely need coverage for these, then these plan options probably aren't a great fit.

Aetna PPO 5,000
The least expensive of the Aetna plans. Exactly the same as Aetna PPO
3,000 but with a higher deductible.

Affordable Coverage for Superheros
You're a superhero with an immune system to match. You don't mind a very high deductible because you rarely go to the doctor. You want an annual physical and you want to be covered for the real big medical bills if you get hit by a bus or get seriously sick. If this sounds like you, then this plan might be a good fit.
How it Works
You pay for all your medical expenses up to the deductible. Once that has been met, Aetna pays 100% of additional in-network expenses. A routine physical is provided annually for just a small copay and no deductible.
The monthly premium is very low, since you'll be assuming responsibility for any healthcare expenses for the year up to the deductible. This is why it's a good idea to use a health savings account to set aside savings for these expenses. Many people fund their health savings account from the money they save on the insurance premiums on this plan.

Aetna PPO 3,000
Middle of the road option with moderate premiums. Exactly the same as
Aetna PPO 5,000 but with a lower deductible.

A Balanced Approach
You're looking for a balance between a lower monthly premium and a lower annual deductible. Maybe you whip out your insurance card twice a year. You don't have a lot of health concerns, except for perhaps your annual physical and the occasional sick visit. You also don't mind a moderate deductible. If this sounds like you, then this plan might be a good fit.

Aetna PPO 2,500
Most expensive Aetna plan with the lowest deductible and unlimited
doctor/specialist visits before the deductible.
If you don't absolutely need this benefit,
then the Aetna PPO 3,000 or 5,000 plans offer a lower monthly premium.
A Hypochondriac's Dream
You love the idea of unlimited doctor visits for just a copay and you don't mind paying a higher monthly premium for this type of coverage. If this sounds like you, then this plan might be a good fit.
How it Works
You pay a small copay for doctor/specialist office visits with no deductible. All other services, (e.g., hospital, lab work, tests, etc.) you pay up to the deductible and coinsurance amounts listed below.
The monthly premium is higher than the other Aetna plans because there's no deductible on doctor visits. Some of the other benefits are actually better on the other Aetna plans, so make sure you really need those office visits before you shell out extra money each month for this plan.

Start dates are only available on the 1st or the 15th of the month. Aetna takes 2-3 weeks to review applications, so don't wait until the last minute!

Finding Doctors in Your Area
Aetna features a national network. You can find in-network doctors and hospitals anywhere in the U.S. You can search the network at Aetna's website. Here's how it works:
- No user name or password is necessary.
- You can search by Zipcode, City, or County.
- Select a provider category (ex: medical providers, hospitals, lab facilities)
- Select a provider Type (ex: primary care, specialists, etc.)
- Select a plan. (This varies state by state. In most cases, it is called Aetna Advantage PPO. If you don't see it listed or are not sure which plan to choose, contact us with your zipcode.)
- Click "Start Search"

Eligibility and Enrollment
Health Proponent is included as a free value-added service with your enrollment in certain health insurance plans through Fractured Atlas (see plan info for details). If you're not enrolled in a qualifying health insurance plan, you can still enroll directly in Health Proponent.
How does it work?
Whenever you have a health care or insurance-related issue, you can call Health Proponent at their toll-free number.
A Personal Health Proponent (PHP), typically a registered nurse supported by medical directors and insurance experts, will be assigned to work with you (and makes the necessary calls) until your issue is resolved, (whether it takes a couple of hours or ten days).
Your medical and personal information is fully protected and held confidential. You will be asked to sign an authorization form permitting Health Proponent to work on your behalf to help resolve your issue.
You and your family can call as often as you'd like 24/7. While normal business hours are Monday-Friday, 8am - 9pm Eastern Time, on call staff is always available during after hours and weekends for assistance during non-business hours.
